1 Personal information
Your name, address, phone, email, plus the date you wrote/typed this resume.
Write your name in
kanji or katakana in the main name field, most Japanese employers expect this. Furigana (ふりがな) is the
phonetic reading in hiragana. The romaji field is just for your reference
and is shown in small text under the name.
Convention is the date you finalized / submitted the resume.

No photo
Standard Japanese ID photo: 3 cm × 4 cm, plain background,
head-and-shoulders, taken within 3 months. We auto-crop and
resize on upload, then store the photo only in your browser
(never uploaded to any server).
2 Education & Work history (学歴・職歴)
Start from your secondary education (high school), or, if you have a long work
history, jump straight to university. Add a row per event in chronological order.
For each school: write the school name plus
入学 (entered) and a
separate row for 卒業 (graduated). For each job: write the company
name plus 入社 (joined). Then add a separate row for leaving:
一身上(いっしんじょう)の都合(つごう)により退社, voluntary resignation. You don't need a specific reason.会社都合(かいしゃつごう)により退社, company dismissal / termination. Use this only if you were actually dismissed. Your 離職票 (resignation notice) records the real reason, so don't fudge it.現在(げんざい)に至(いた)る, "still working" (use this for your current job; goes on its own row with no specific year/month action).

List: JLPT (the date you passed and the level, score is NOT needed),
Japanese driver's license (date received), plus any other
professional certifications (AWS, Cisco, accounting, real estate, etc.). Use the

4 Motivation, self-PR & strengths (志望の動機、特技、自己PR)
There's a standard template Japanese applicants use. Direct translation:
"I applied to your company because of X. I was a Y at my
previous company and delivered results like Z. Therefore I want to use
my skills as Y and contribute to your company."
5 Personal wishes (本人希望記入欄)
Usually not necessary. The safe default is
貴社規定に従います。
("I will follow your company's rules."), this is what most applicants write.
You CAN include constraints like "I can only work 9am–6pm" or "weekdays only",
but be ready to defend the request in your interview.
